They analyzed the data from 3199 American adults with type-2 diabetes; the average follow-up period was 12.6 years [Qiu 2021].<\/strong><\/p>\n<p><strong>During that follow-up period, 1693 deaths were documented, including 425 heart disease deaths [Qiu 2021].<\/strong><\/p>\n<h5>Highest Quartile of Serum Selenium Compared to Lowest Quartile<\/h5>\n<p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34664061\/\" target=\"_blank\" rel=\"noopener\"><strong><em>Individuals in the highest quartile of serum selenium concentration had significantly lower all-cause mortality rates and significantly lower heart disease mortality rates when compared with individuals in the lowest quartile of serum selenium concentration<\/em><\/strong><\/a> [Qiu 2021].<\/p>\n<h5>Dose-response Relationship Between Serum Selenium and Mortality<\/h5>\n<p>Moreover, the researchers observed a linear dose\u2013response relation<br \/>\nbetween serum selenium (range: 89\u2013182 mcg\/L) and mortality [Qiu 2021].<\/p>\n<p>The upper range of serum selenium concentration, up to 182 mcg\/L, is considered a relatively high selenium concentration, but it is still within the<br \/>\nnormal physiologic range [Qiu 2021].<\/p>\n<p>Professor Lutz Schomburg in Berlin explains that higher selenium status may be found in diabetes patients because <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31823341\/\" target=\"_blank\" rel=\"noopener\"><strong><em>diabetes seems to cause increased Selenoprotein P (SELENOP ) biosynthesis in the liver and seems to raise the level of circulating SELENOP and selenium<\/em><\/strong><\/a> [Schomburg 2020].<\/p>\n<p>He explains that, under normal conditions, insulin decreases SELENOP biosynthesis and secretion. In cases of insulin resistance, however, this inhibition declines, causing increased SELENOP levels and higher circulating selenium concentrations [Schomburg 2020].<\/p>\n<h5>Control of Potential Confounding Variables in the Diabetes study<\/h5>\n<p>In the <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34664061\/\" target=\"_blank\" rel=\"noopener\"><em><strong>Qiu study<\/strong><\/em><\/a>, the significantly reduced risks of all-cause death and heart disease death were observed when the researchers stratified the data and adjusted for potential confounding variables [Qiu 2021]:<\/p>\n<ul>\n<li>age<\/li>\n<li>BMI<\/li>\n<li>diabetes duration<\/li>\n<li>HbA1c concentrations<\/li>\n<li>physical activity<\/li>\n<li>race<\/li>\n<li>sex<\/li>\n<li>smoking status<\/li>\n<\/ul>\n<h5>Oxidative Stress and Inflammation in Type-2 Diabetes<\/h5>\n<p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31412712\/\" target=\"_blank\" rel=\"noopener\"><strong><em>Diabetes can induce oxidative stress and inflammatory activation<\/em><\/strong><\/a> [Qiu 2021].<\/p>\n<p>Both oxidative stress and inflammation are enhanced in chronic heart failure [Aimo 2020].<\/p>\n<p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26662217\/\" target=\"_blank\" rel=\"noopener\"><em><strong>Antioxidants<\/strong><\/em><\/a> and\/or <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26375288\/\" target=\"_blank\" rel=\"noopener\"><strong><em>anti-inflammatory agents<\/em><\/strong><\/a>, including the antioxidant seleno-enzymes such as the glutathione peroxidases and the thioredoxin reductases, may be useful add-on therapies for individuals at high risk for or with heart failure [Alehagen 2015].<\/p>\n<p>In the BioStat-CHF cohort study, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31808274\/\" target=\"_blank\" rel=\"noopener\"><strong><em>heart failure patients with serum selenium less than 70 mcg\/L had worse symptoms, worse exercise capacity, worse quality of life, and a significantly higher risk of all-cause mortality<\/em><\/strong><\/a>. Heart failure patients with serum selenium less than 100 mcg\/L had nearly as poor symptoms and quality of life [Bomer 2020].<\/p>\n<p>In a cohort study from China, researchers found that <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31158596\/\" target=\"_blank\" rel=\"noopener\"><strong><em>baseline plasma selenium concentrations were inversely associated with cardiovascular disease risk in 3897 patients with diabetes<\/em> <\/strong><\/a>over an average follow-up period<br \/>\nof 6.2 years [Long 2019].<\/p>\n<h5>Conclusion<\/h5>\n<p>Higher serum selenium concentrations have been associated with lower all-cause mortality and lower heart disease mortality among individuals with type 2 diabetes.<\/p>\n<h5>Sources<\/h5>\n<p>Aimo A, Castiglione V, Borrelli C, Saccaro LF, Franzini M, Masi<br \/>\nS, Emdin M, Giannoni A.
