The effect is especially evident in adults with diabetes or insulin resistance. The effect of selenium supplementation in women with polycystic ovary syndrome and in postmenopausal women is less unequivocal. Moreover, studies show that selenium supplementation of individuals with replete selenium status does not make sense [Morales-Ju\u00e1rez 2026].<\/strong><\/p>\n<p><strong>What other effects, in addition to improving <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/41762697\/\" target=\"_blank\" rel=\"noopener\">insulin sensitivity<\/a>, does selenium supplementation have in individuals with diabetes or insulin resistance? <\/strong><\/p>\n<p><strong>Selenium supplementation is associated with enhanced antioxidant defenses and with lower levels of the inflammation bio-marker hs-CRP in adults with diabetes and insulin resistance. This association is important to note because type 2 diabetes is characterized by oxidative stress and chronic inflammation. The benefits of the supplementary selenium have mostly occurred in studies with intervention durations of 12\u2009weeks or longer [Morales-Ju\u00e1rez 2026].<\/strong><\/p>\n<p>In an earlier <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31148299\/\" target=\"_blank\" rel=\"noopener\">systematic review<\/a> of four randomized controlled trials enrolling 241 participants, Strozyk et al observed that, in individual studies, selenium supplementation had significantly reduced fasting insulin levels, HOMA-insulin resistance, and HOMA-estimated B cell function. This indicates that the selenium was associated with more efficient glucose control with less insulin production [Strozyk 2019].<\/p>\n<h5>What Do Intervention Studies of Patients with Diabetes Show?<\/h5>\n<h6>Type 2 Diabetes Patients with Coronary Artery Disease<\/h6>\n<p>In patients with type 2 diabetes and <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26743526\/\" target=\"_blank\" rel=\"noopener\">coronary artery disease<\/a>, supplementary selenium at the rate of 200 micrograms per day for an eight-week period improved the following test values [Farrokhian 2016]:<\/p>\n<ul>\n<li>serum insulin levels associated with insulin resistance (HOMA-IR)<\/li>\n<li>insulin sensitivity (QUICKI)<\/li>\n<li>total antioxidant capacity<\/li>\n<li>inflammatory markers (hs-CRP)<\/li>\n<\/ul>\n<p>Note: HOMA-IR and QUICKI are indices calculated from fasting glucose and fasting insulin levels. They estimate insulin resistance and insulin sensitivity. When insulin resistance is higher, more insulin is needed to maintain normal glucose levels.<\/p>\n<h6>Type 2 Diabetes Patients Undergoing Hemodialysis<\/h6>\n<p>In patients with diabetes receiving hemodialysis, the administration of 200 micrograms of selenium daily for 24 weeks was associated with a significant reduction in serum insulin levels, insulin resistance, and C-reactive protein, compared with placebo. The researchers also observed a significant increase in insulin sensitivity, compared with the placebo [Salimian 2022].<\/p>\n<h6>Type 2 Diabetes Patients with Congestive Heart Failure<\/h6>\n<p>In patients with congestive heart failure, 12 weeks of daily supplementation with 200 micrograms of selenium was associated with reductions in serum insulin and HOMA-IR [Raygan 2018].<\/p>\n<h5>What Explains Correlation Between Selenium Status and Diabetes Risk?<\/h5>\n<p>Prof. Lutz Schomburg has argued that observed correlation between higher selenium status and diabetes is due to <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31823341\/\" target=\"_blank\" rel=\"noopener\">reverse causation<\/a>, i.e., having insulin resistance or diabetes leads to increases in selenium status rather than selenium causes diabetes [Schomburg 2020].<\/p>\n<ol>\n<li>From the available selenium, the liver produces selenoprotein P, the primary selenium transporter in the blood.<\/li>\n<li>Normally, insulin suppresses selenoprotein P production. However, in insulin resistance or type 2 diabetes, this suppression is diminished.<\/li>\n<li>Consequently, selenoprotein P synthesis increases, increasing circulating levels of selenium.<\/li>\n<li>For this reason, individuals with insulin resistance or diabetes may show higher selenium status.<\/li>\n<\/ol>\n<p>Thus, in observational studies, it may appear that high selenium status is causing type 2 diabetes. These appearances may be deceiving. More likely, it is the other way around: diabetes\/insulin resistance is causing serum selenium levels to rise [Schomburg 2020].<\/p>\n<h5>Conclusions: Selenium Status and Diabetes Risk<\/h5>\n<p>Basically, 200 micrograms\/day selenium is a commonly used dose associated with improvements in glucose metabolism for patients with type 2 diabetes, gestational diabetes, and heart failure [Morales-Juarez 2026].<\/p>\n<p>Both selenium deficiency and excess of selenium can be harmful. Accordingly, supplement dosing must be guided by baseline selenium status, patient sex, patient comorbidities, and patient hormonal status [Morales-Juarez 2026].<\/p>\n<p>Discrepancies in the outcomes of selenium supplementation seem to reflect differences in study design: differences in baseline selenium status, dosage, and participant characteristics [Morales-Juarez 2026].<\/p>\n<p>Correlations between selenium status and diabetes incidence most likely reflect diabetes-induced increases in selenoprotein P and circulating selenium, not a diabetogenic effect of selenium [Schomburg 2020].<\/p>\n<h5>Sources<\/h5>\n<p>Amirani E et al.
