sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><figcaption id=\"caption-attachment-588\" class=\"wp-caption-text\"><strong>In our thinking about our diet and our fitness, scientific research must be our highest authority.\u00a0 The results from randomized controlled studies are the best evidence for the possible existence of a cause-effect relationship between treatment with a selenium (or Coenzyme Q10) supplement and beneficial health outcomes.<\/strong><\/figcaption><\/figure>\n<p><strong>Recently, I did a search of the <a href=\"https:\/\/www.nlm.nih.gov\/pubs\/factsheets\/dif_med_pub.html\" target=\"_blank\" rel=\"noopener noreferrer\"><em>Medline<\/em> database<\/a> on the subject of selenium supplementation and its effect or non-effect on glucose metabolism, insulin resistance, glycemic control, etc.\u00a0 I limited my search to reports of data from randomized, controlled trials.<\/strong><\/p>\n<p><strong>Altogether, the Medline search yielded 48 hits.\u00a0 Reading through the abstracts of the 48 journal articles, I was able to eliminate 30 references from consideration.\u00a0 These 30 eliminated studies were studies that included all of my search terms but did not actually measure the effect of an intervention with selenium on some aspect of glucose metabolism.<\/strong><\/p>\n<p><strong>18 good selenium supplementation studies<\/strong><br \/>\nI was left with 18 good intervention studies that met my search criteria.\u00a0 I read these studies and separated them into three distinct categories related to the effect of selenium supplementation on glucose metabolism and insulin sensitivity:<\/p>\n<ul>\n<li>Beneficial effect of selenium supplementation (n=6)<\/li>\n<li>No effect of selenium supplementation (n=9)<\/li>\n<li>Not beneficial effect of selenium supplementation (n=3)<\/li>\n<\/ul>\n<p><strong>Selenium supplementation beneficial for glucose metabolism<\/strong><br \/>\nThe six studies showing beneficial effects of selenium supplementation on glucose metabolism and\/or insulin sensitivity enrolled a combined 402 patients.\u00a0 All six studies were conducted on Iranian patients.<\/p>\n<p>Basically, these six studies showed the following health benefits associated with the selenium supplementation:<\/p>\n<ul>\n<li>decreased plasma glucose levels<\/li>\n<li>decreased serum insulin levels<\/li>\n<li>decreased plasma HOMA-B levels<\/li>\n<li>decreased plasma HOMA-IR levels<\/li>\n<\/ul>\n<p><em>Note<\/em>:\u00a0 The HOMA (homeostasis model assessment) measurements are based on plasma levels of fasting glucose and insulin.\u00a0 HOMA scores have been validated for quantifying insulin resistance [Song].<\/p>\n<p><strong>Selenium supplementation with no effect on glucose metabolism<\/strong><br \/>\nThe nine studies showing no effect of selenium supplementation \u2013 neither positive nor negative \u2013 on glucose metabolism enrolled a total of 8568 patients, all of them study participants in the United Kingdom, the United States, or France.<\/p>\n<p>In these nine studies, selenium supplementation was not associated with any significant changes:<\/p>\n<ul>\n<li>in plasma or serum glucose levels<\/li>\n<li>in insulin sensitivity measures<\/li>\n<li>in bio-markers of insulin resistance<\/li>\n<\/ul>\n<p><strong>Selenium supplementation with non-beneficial effect on glucose metabolism<\/strong><br \/>\nThe three studies that showed an undesirable effect of selenium supplementation on glucose metabolism enrolled a total of 1315 patients and study participants.\u00a0 The patients in two of the studies (n = 113) were of Iranian origin.\u00a0 The participants in the third study (n=1202) lived in the United States.<\/p>\n<ul>\n<li>One Iranian study (n=60) reported increased insulin resistance [Hosseinzadeh].<\/li>\n<li>The other Iranian study (n=53) reported increased fasting plasma glucose [Faghihi].<\/li>\n<li>The big American study (n=1202) was the <em>post-hoc<\/em> study by Dr. Saverio Stranges of the data from the Nutritional Prevention of Cancer (NPC) study.<\/li>\n<\/ul>\n<p>The NPC study data show a strong association between daily supplementation with 200 micrograms of a high-selenium yeast preparation and <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/8971064\" target=\"_blank\" rel=\"noopener noreferrer\">significant reductions in total cancer mortality<\/a>, total cancer incidence, and prostate cancer incidence [Clark].<\/p>\n<p>Dr. Stranges reported finding an \u201cexposure-response gradient\u201d across tertiles of baseline plasma selenium concentrations.\u00a0 He reported an increased risk for type 2 diabetes in the highest tertile only, the tertile with a baseline plasma selenium level greater than 121.6 micrograms per liter.<\/p>\n<p>As limitations to his study, Dr. Stranges did note that diabetes was a secondary outcome in the NPC trial and that the diagnoses of diabetes were self-reported [Stranges].<\/p>\n<p><strong>U-shaped curve in the selenium-glucose metabolism relationship?<\/strong><br \/>\nThe relationship between selenium and glucose metabolism is clearly a complex one.\u00a0 Professor Rayman, University of Surrey, Guildford, United Kingdom, has suggested that there may be a <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23597503\" target=\"_blank\" rel=\"noopener noreferrer\">U-shaped relationship<\/a> between the levels of certain selenoproteins and glucose metabolism or insulin resistance [Rayman].<\/p>\n<p>In that case, it might be that both low levels of selenium and high levels of selenium over a longer period could have harmful effects.<\/p>\n<p>The <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20181815\" target=\"_blank\" rel=\"noopener noreferrer\">protective range<\/a> for plasma selenium status seems to be <strong><em>120-170 micrograms per liter<\/em><\/strong> [Hurst].<\/p>\n<p>A systematic review and meta-analysis of five <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27142520\" target=\"_blank\" rel=\"noopener noreferrer\">observational studies<\/a> (note: not intervention studies) published in 2016 has also suggested a U-shaped relationship between selenium status and the risk of type-2 diabetes.\u00a0 Populations with relatively low levels and relatively high levels of serum selenium were seen to be at increased risk [Wang].<\/p>\n<p><strong>Appropriate level of selenium supplementation<\/strong><br \/>\nGiven what we know about the beneficial effects of optimal selenium levels, I choose to get my blood serum selenium concentration tested.<\/p>\n<ul>\n<li>beneficial effects on the prevention of cancer<\/li>\n<li>beneficial effects on the prevention of thyroid disorders<\/li>\n<li>beneficial effects on the strengthening of the immune system<\/li>\n<li>beneficial effects on the prevention of <a href=\"http:\/\/www.q10facts.com\/coenzyme-q10-and-selenium-partners-against-heart-disease\/\" target=\"_blank\" rel=\"noopener noreferrer\">heart disease<\/a><\/li>\n<\/ul>\n<p>Then, with the test results in hand, I can decide which level of daily selenium supplementation \u2013 50 or 100 or 200 micrograms daily \u2013 is appropriate for me.<\/p>\n<p><strong>Limitations of the available research<\/strong><br \/>\nThe randomized controlled studies that have been done on the effect of selenium supplementation on glucose metabolism have been done on samples with much heterogeneity with respect to age, gender, ethnic background, and medical condition.<\/p>\n<p>Furthermore, most of the data come from studies in which the effect on glucose metabolism was not the primary outcome measured.<\/p>\n<p>We need more and better studies.\u00a0 However, we are not likely to see funding for large well-designed studies because selenium is not a substance that can be patented. \u00a0The best we can do is aim to keep our selenium status in the estimated protective range [Hurst].<\/p>\n<p>Sources<\/p>\n<p>Studies showing a beneficial
